Learning Disability Register
The Learning Disability Register is a list of people who have a learning disability at the GP Practice.
A person does not need to have a special assessment or test to be on the learning disability register.

Health Checks
Everyone with a learning disability can ask for a health check every year at their GP surgery. A health check can help you keep well.

Reasonable Adjustments
Reasonable adjustments are small changes that can help people with a learning disability be treated equally. It is important that you tell staff if you need a reasonable adjustment.

Examples of small changes
- A quiet place to sit if you find noise difficult or don’t like bright lights.
- A appointment time that suits your needs.
- A double appointment that gives you extra time.
- Information in a way that you can understand.
- Having someone with you to support you when you see the doctor.
Reasonable adjustment flag
The Reasonable Adjustments Digital Flag is like a special note that tells doctors and nurses that a person needs extra help or changes to their care. This note is shared with all health services to make sure the person gets the right support wherever they go.
If you do not want the flag to appear in your record please inform the practice.
